Rebar placement, cover, and lap splice lengths are specified by the structural engineer. Inspectors verify bar size, spacing, and cover before pour. Bending rebar in the field requires approval — never bend and rebend a bar in the same location.

Rule of Thumb
Minimum concrete cover over rebar is typically 1.5 inches for interior slabs, 2 inches for beams and columns, and 3 inches for concrete cast against earth.
